When should I see a urologic oncologist?
If you have an abnormal PSA, blood in the urine, a palpable testicular lump, persistent lower back pain with no apparent cause, or a family history of prostate, kidney or bladder cancer, a specialist assessment is worthwhile. Men over 50 (or 45 with a family history) should have annual PSA screening in line with SBU and AUA guidelines.
Does a high PSA mean prostate cancer?
Not necessarily. PSA is a marker, not a diagnosis. It can be elevated due to benign hyperplasia, prostatitis, urinary tract infection, recent prostate manipulation or the natural growth of the prostate. The decision to investigate with a biopsy takes into account how fast the PSA is rising, age, digital rectal exam, multiparametric MRI and family history. An individualized assessment is essential — do not draw conclusions on your own.
I have been diagnosed with prostate cancer. Do I need surgery?
Not always. Low-risk tumors can be monitored with active surveillance, and selected cases of localized disease can be treated with focal therapy (HIFU or electroporation). Robotic surgery and radiotherapy are indicated according to the tumor's risk, your age and your priorities. The decision is shared and based on AUA, EAU and NCCN guidelines.
Can I book a consultation just for a second opinion?
Yes, and it is a common reason for a consultation. Bring (or send in advance via WhatsApp) your PSA, MRI, biopsy report and other tests. Dr. Bruno reviews the case with you and explains, in clear language, which options make sense.
What is the difference between a transperineal and a transrectal prostate biopsy?
In a transperineal biopsy, the needle enters through the skin of the perineum (between the scrotum and the anus), without passing through the rectum. This substantially reduces the risk of infection and allows better sampling of the anterior regions and apex of the prostate. In a transrectal biopsy, the needle passes through the rectal wall. The European Association of Urology (EAU) guidelines preferentially recommend the transperineal route.
Does a transperineal biopsy hurt? What anesthesia is used?
The procedure is performed under sedation combined with local anesthesia with a perineal block, for your comfort. It usually takes 20 to 40 minutes, and you go home the same day. Mild discomfort and blood in the urine or semen for a few days may occur, which is expected.
Do I need an MRI before a prostate biopsy?
In most cases, yes. Guidelines recommend multiparametric MRI before biopsy: it locates suspicious areas (PI-RADS classification), allows the needles to be targeted by image fusion and, in some patients, helps avoid an unnecessary biopsy.
How long does it take to get prostate biopsy results?
The pathology report is usually ready in 7 to 10 days. The result is explained at a consultation: if cancer is found, we discuss the grade (Gleason/ISUP), the extent, the need for staging tests such as PET-PSMA and the treatment options.
How long does a consultation last?
Initial uro-oncology consultations last on average 45 to 60 minutes, because they require a detailed review of tests, a physical examination, a discussion of family history and the shared development of the treatment plan. Follow-up visits last in proportion to the complexity of the case.
Do you offer telemedicine consultations?
Yes. We offer telemedicine consultations in accordance with Brazilian Federal Council of Medicine (CFM) Resolution 2,314/2022 — especially for second opinions, clinical follow-ups, review of test results and patients from other cities or countries who need an initial assessment before traveling to São Paulo.
Is robotic surgery covered by health insurance?
